Start Your Journey with Omnipresent Supportive Care

Eligibility Criteria

To qualify for enrollment at Omnipresent Supportive Care LLC, participants must meet the following requirements:

• Be 18 years of age or older

• Have a diagnosis of an intellectual, developmental, or mental health disability

• Be able to safely participate in a non-medical, community-based day program

• Have a responsible guardian or support contact (if applicable)

• Complete all required forms and assessments before starting services

What to Expect

Once we receive your application, a member of our admissions team will contact you to schedule an orientation meeting. During this meeting, we’ll review your individual goals, care needs, and available program options to ensure Omnipresent is the right fit for you or your loved one.

You’ll also:

• Meet our staff and tour the facility

• Review your service plan and schedule

• Learn about daily routines, activities, and expectations

• Discuss payment options and transportation support

 

​Accepted Payment Options

Omnipresent Supportive Care LLC proudly accepts:

• Medicaid (Partners Health Management & DHHS Services)

• Private Pay (self-pay option)

Our staff will help guide you through verification and ensure your service plan aligns with funding requirements or private-pay terms.

Required Documents

Before services begin, please have the following documents available:

• Valid photo ID or Medicaid card

• Proof of diagnosis or current care plan

• Guardian contact information (if applicable)

• Emergency contact information

• Any behavioral or medical support documentation (if relevant)
