Omnipresent Supportive Care LLC provides structured and individualized services designed to support independence, daily engagement, and community involvement. All services are delivered based on each participant’s authorized plan and level of need.

DAY ACTIVITY PROGRAM (27G.5400)

Our Day Activity Program provides structured, supervised services in a group setting. Participants engage in daily activities that promote social interaction, skill-building, and routine in a safe and supportive environment.

Services are provided in a licensed facility setting.

COMMUNITY LIVING & SUPPORTS (CLS)

Community Living & Supports (CLS) provides individualized support to assist participants with daily living skills and community engagement. Services are delivered based on each participant’s needs and goals.

What this includes:

1:1 individualized support
Assistance with daily living skills
Community outings and engagement

Communication and social support
Goal-based support aligned with the Individual Support Plan (ISP)

Services may be provided in the home or community setting.

LIFE SKILLS & TRANSITION SUPPORT

We support individuals in developing skills that promote independence and assist with successful transitions into adulthood and community life.

Daily living skills (hygiene, routines)
Communication and social skills
Task completion and following directions
Pre-vocational and employment readiness skills

These supports are incorporated into daily programming and individualized services.

HOW SERVICES ARE PROVIDED

All services are provided in accordance with the participant’s Individual Support Plan (ISP) and are authorized through Medicaid or other applicable funding sources.

Services are individualized to each participant and delivered based on assessed needs, goals, and level of support required.
